Authorization for Release of Information

This lets your student's school share their disability documentation with NDCS, so we can verify eligibility for Vocational Rehabilitation youth programs.

What you are signing

  • You are authorizing the student's school to talk with NDCS and release the records you check — a 504 plan, IEP, IPE or other acceptable documentation.
  • The purpose is narrow: to verify the student has a documented disability so they can take part in the Division of Vocational Rehabilitation youth work readiness, work experience and career camp programs.
  • The authorization expires one year after the date signed, and you may withdraw your consent at any time.
  • A parent or guardian signs if the student is under 18. You can sign with a finger, a mouse, or by typing your name.

Most families reach this form after starting the youth interest form. If you are not sure whether you need it, ask us before signing.
