Youth Work-Based Learning interest form

This is the first step toward a Pre-ETS referral. It takes about ten minutes, and Kathryn reads every one personally.

Before you start

  • Who this is for: students ages 14–21 with a documented disability who are still in school.
  • Have handy: the student's date of birth, school name, and a copy of the IEP, IPE, 504 plan or other documentation to upload. If you do not have documentation yet, say so on the form — we help families get it.
  • A signature is required: the student signs, and a parent or guardian gives permission for the referral to Vocational Rehabilitation. You can sign with a finger, a mouse, or by typing your name.
